Personal Data / Subject Access Request

Before you make a request

You can use this form to request data about yourself and/or on behalf of someone else.

You will be required to provide some supporting documentation as set out below.

If you are requesting your own personal data, we require proof of your identity and address. This could be your passport, driving licence or travel pass and a utility bill or bank statement showing your current address (no more than 3 months old).

If you are seeking information on behalf of someone else, including children over 12, then you must also provide:

• Proof of identity and address of the person whose data you require (the data subject)
• Proof of identity of the person requesting the data
• Authority to act on behalf of the data subject. This should be either a signed letter from the data subject giving authority, or a letter signed by a doctor or legal professional confirming that the data subject is incapacitated and therefore cannot make the request themselves.

You will be asked to send us copies of the documents via e-mail, or post after submitting the request.

Please note we will not process your request until we receive all relevant documentation.
